                                          4. CONFIRMATION AND CONSENT TO INQUIRY
              An  inquiry  may  only  be
              conducted with the consent of
              the employee, or in accordance
                                                               (Name of Employee)
              with a collective agreement, or
              where an employee,  earning
              more than the threshold, has  confirm that I have been advised of the allegations against me; and
              consented to the holding of the
              inquiry  in  a  contract  of     (a)  I consent to the process: or
              employment.
                                               (b) am bound by a collective agreement providing for the inquiry. A copy of the
              BARGAINING    COUNCIL'S             collective agreement is attached; or
              COLLECTIVE AGREEMENT
                                               (c) I earn more than the threshold and have consented to the process in my
              Employers  requesting  the          contract of employment. A copy of the contract of employment is attached
              Council to appoint an arbitrator    hereto.
             to  conduct  an  inquiry  must
              submit the following documents:
                   Proof of membership of
                   an        employer's
                   organisation party to the  EMPLOYEE SIGNATURE
                   Council,
                   Proof of registration and
                                          5. PAYMENT OF FEES:
                   payment of levies to the
                   Council.
                                          Proof of payment of the prescribed fee is attached.
