Gauteng GAMBLING ACT, 1995

APPLICATION FOR AN AMENDMENT OF A SITE OPERATOR LICENCE TO INCREASE THE NUMBER OF
                                                          LPM’S

•   Mitchell  Street  Restaurant  &  Bar  (Pty)  ltd trading  as Mitchell  Street  Restaurant  &  Bar  of 252  Charlotte
                   Maxeke Street, Pretoria West;
                •   Phumelela Gaming and Leisure Limited trading as TAB Mabopane of Corner Commando and Maraisburg
                   Road, Industria West, Pretoria;
                •   Marco Bezuidenhout trading as Cavemans Pub and Grill of Shop 01 Connoisseur Building, Corner of Gerrit
                   Maritz and Jack Hindson Street, Pretoria North;

Intend submitting applications to the Gauteng Gambling Board for an amendment of gaming machine licenses from
                two to five machines. These applications will be open for public inspection at the offices of the Board from 07
                August 2018.
                Attention is directed to the provision of Section 20 Gauteng Gambling Act, 1995 which makes provision for the
                lodging of written representations in respect of the application. Such representations should be lodged with the
                Chief Executive Officer, Gauteng Gambling Board, Private Bag 15 Bramley, 2018 within one month from 07 August
                2018.

APPLICATION FOR A CHANGE OF FLOOR PLAN

•   Phumelela Gaming and Leisure Limited trading as TAB Centurion of Office 101, Dorinkloof Mall,
                       Dorinkloof Centurion;
                   •   Phumelela Gaming and Leisure Limited trading as TAB Tembisa of Shop 101 A, Tembisa Shopping
                       Mall, Erven 232, 233, 234, Igqagqa Section, Tembisa;

Intend submitting applications to the Gauteng Gambling Board for change of floor plan. These applications will be
                open for public inspection at the offices of the Board from 07 August 2018.

Attention is directed to the provision of Section 20 Gauteng Gambling Act, 1995 which makes provision for the
                lodging of written representations in respect of the application. Such representations should be lodged with the
                Chief Executive Officer, Gauteng Gambling Board, Private Bag 15 Bramley, 2018 within one month from 07 August
                2018.
                Any person submitting representations should state in such representation whether or not they wish to make oral
                representations at the hearing of the application.
