IMPORTANT NOTICE
              The Government Printing Works will not be held responsible for faxed documents not received due
              to errors on the fax machine or faxes received which are unclear or incomplete. Please be advised
              that an “OK” slip, received from a fax machine, will not be accepted as proof that documents were
              received by the GPW for printing. If documents are faxed to the GPW it will be the sender’s respon-
              sibility to phone and confirm that the documents were received in good order.
              Furthermore the Government Printing Works will also not be held responsible for cancellations and
              amendments which have not been done on original documents received from clients.

NOTICE 1316 OF 2014

APPLICATION FOR AMENDMENT OF TOTALIZATOR LICENCE

Notice is hereby given that Phumelela Gaming and Leisure Limited (Tab Carletonville)
                    intends to submit an application to the Gauteng Gambling Board for an amendment of a
                    Totalizator licence to relocate the outlet from Shop 7B, Gateway Shopping Centre, Cnr
                    Osmium & Annan Service Road, Carletonville to Shop No 3, Ex KFC Building, ERF
                    010132900, Cnr Gold & Ada Streets, Carletonville.

The application will be open for public viewing at the offices of the Board
                    from the 09th May 2014.

Attention is directed to the provision of section 20 of the Gauteng
                    Gambling Act, 1995 which makes provision for the lodging of written
                    representations in respect of the application.

Such representations should be lodged with the Acting Chief Executive Officer, Gauteng
                    Gambling Board, Private Bag 15, Bram ley, 2018 or 125 Corlett Drive, Bram ley,
                    Johannesburg within one month from the 09th May 2014.

Persons wishing to make objections or comments on this application shall do so in writing
                    to the Authority within the period mentioned above and state whether or not they wish
                    to make oral representations at the hearing of the application.
