APPLICATION  FEE:  APPLICATION FOR A  LICENCE FOR PHARMACY
                                                       PREMISES

The  Director-General:  Health,  intends to  publish  the  following amendments to  fees
                    payable  on application of a  Pharmacy  Premises  Licence issued in  terms  of Section
                    22  and  22A  of the Pharmacy Act,  1974  (Act No.  53 of  1974)  in the  schedule.

Interested  persons  are  invited  to  submit  any  substantiated  comments  in  writing  to
                    the  proposed  application  fees,  to  the  Director-General:  Health,  Private  Bag  X828,

Pretoria,  0001 (for  the  attention  of  the  Chief  Director:  Sector-Wide  Procurement)
                    within  30 days from date of publication of this notice.

SCHEDULE

The  following  application  fees  shall  be  payable  to  the  Council.  The  fees  include

Value  Added Tax (VAT):

a)     A  fee to accompany the application form:  R1 ,857-98;
                    b)     Request  for  the  re-print  of  a  licence  will  be  subject  to  an  application  fee  to

recover  administrative  costs:  R300-00,  payable  to  the  National  Department
                           of Health.
